General Eligibility Requirements

  • Applicants must be a U.S. Citizen, Permanent Resident status does not qualify to apply to the ADA Foundation Scholarship Programs;
  • Criteria for eligibility for an award is determined by the applicant’s demonstrated financial need, academic achievement, biographical sketch questionnaire and two completed reference forms;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate financial need;
  • Applicants must have an accumulative grade point average of 3.5 based on a 4.0 scale.

Application Process

  • Only schools accredited by the Commission on Dental Accreditation of the American Dental Association are eligible to participate in the ADA Foundation’s Scholarship Program;
  • School officials may contact the ADA Foundation by phone at 312-440-2763 or by e-mail: famularor@ada.org to request the scholarship application form;
  • Students must contact the associate dean for student affairs/program director or financial aid officer at the school to request a scholarship application form as the forms are disbursed by school officials;
  • The appropriate school officials should select its top two students as the ADA Foundation will only accept two applications from each school for each scholarship program to consider.

Applicants must submit to their schools:

  • Application form that is typed, completed and signed by school officials;
  • Completed application form, including the Academic Achievement Record Form and Financial Needs Assessment Form, which are a part of the application form, and signed by school official;
  • A copy of the school’s letter of acceptance, if entering first-year student;
  • Two completed reference forms, sealed and signed on the back flap of the envelopes by the referrers (required forms must be used which are a part of the scholarship application form);
  • Typed, biographical sketch questionnaire (required form must be used which is a part of the scholarship application form);

Schools must submit their top two (2) completed applications to the ADA Foundation by the deadline date for consideration.

Awards

The following scholarships are awarded each year; however, this is based on available funding:

15 Awards $1,000 each award $15,000 total funding
Dental Assisting

Notification of funding decisions will be made after December 31.

Criteria for Each Scholarship Program

Dental Student Scholarship

The maximum annual award for the Dental Student Scholarship is $2,500.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
  • Applicants must be an entering second year student at the time of application and currently attending or enrolled at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate a minimum financial need of $2,500;
  • Applicants must have a minimum accumulative grade point average of 3.0 based on a 4.0 scale;
  • Two reference forms from two dental school representatives (i.e., professor or academic advisor) in support of the application must be submitted as part of the application form.

Minority Dental Student Scholarship

The maximum annual award for the Minority Dental Student Scholarship is $2,500.

The Minority Dental Student Scholarship Program mirrors that of the Dental Student Scholarship. Certain minority groups have been identified as being underrepresented in dental school enrollment. At this time, African American, Hispanic and Native American students are eligible to apply for this scholarship.

The ADA Foundation’s Minority Dental Student Scholarship Program is funded in part by Harry J. Bosworth Company, Colgate-Palmolive, Proctor & Gamble and Sunstar Americas.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
  • Applicants must be an entering second year student at the time of application and currently attending or enrolled at a dental school accredited by the Commission on Dental Accreditation of the American Dental Association;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate a minimum financial need of $2,500;
  • Applicants must have a minimum accumulative grade point average of 3.0 based on a 4.0 scale;
  • Two reference forms from two dental school representatives (i.e., professor or academic advisor) in support of the application must be submitted as part of the application form.

Allied Dental Health Scholarships

Dental Hygiene Scholarship Program

The maximum annual award for the Dental Hygiene Scholarship is $1,000.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
  • Applicants must be an entering final year student at the time of application and currently attending a dental hygiene program accredited by the Commission on Dental Accreditation of the American Dental Association;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate a minimum financial need of $1,000;
  • Applicants must have a minimum accumulative grade point average of 3.5 based on a 4.0 scale;
  • Two reference forms from two dental hygiene program representatives in support of the application must be submitted as part of the application form.

Dental Assisting Scholarship Program

The maximum annual award for the Dental Assisting Scholarship is $1,000.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
  • Applicants must be an entering student at the time of application and enrolled in a dental assisting program accredited by the Commission on Dental Accreditation of the American Dental Association;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate a minimum financial need of $1,000;
  • Applicants must have a minimum accumulative grade point average of 3.5 based on a 4.0 scale;
  • Two reference forms, one from a dentist or dental assisting representative and/or one from a school representative in support of the application must be submitted as part of the application form.

Dental Laboratory Technology Scholarship Program

The maximum annual award for the Dental Laboratory Technology Scholarship is $1,000.

The ADA Foundation’s Dental Laboratory Technology Scholarship Program is funded in part by Handler Manufacturing, Inc.

  • Applicants must be a U.S. Citizen (permanent resident status does not qualify);
  • Applicants must be an entering final year student at the time of application and currently attending a dental laboratory technology program accredited by the Commission on Dental Accreditation of the American Dental Association;
  • Applicants must be enrolled as a full-time student, a minimum of 12 credit hours;
  • Applicants must demonstrate a minimum financial need of $1,000;
  • Applicants must have a minimum accumulative grade point average of 3.5 based on a 4.0 scale;
  • Two reference forms from two dental laboratory technology program representatives in support of the application must be submitted as part of the application form.
